    Patient education and disease activity: A study among rheumatoid arthritis patients

    Objective: To determine whether patients experiencing high disease activity derive more benefit from patient education than those experiencing low disease activity. - \ud Methods: Data from a randomized study on the effects of a program of patient education were analyzed retrospectively. Four subgroups were studied: the high disease activity subgroup of patients who had participated in the educational program, the complementary low disease activity subgroup, the high disease activity subgroup of controls, and its low disease activity complement. Patients with erythrocyte sedimentation rate >28 mm/first hour were classified as having high disease activity. Effects on frequency of physical exercises, endurance exercises, and relaxation exercises and effects on health status (Modified Health Assessment Questionnaire, Dutch Arthritis Impact Measurement Scales [AIMS]) were measured. - \ud Results: There were no significant differences between the adherence parameters of the various pairs of groups. Four months after the educational program began, anxiety and depression scores on the Dutch-AIMS had increased among participating patients who were experiencing high disease activity and decreased among those who were experiencing low disease activity. - \ud Conclusions: Patients experiencing high disease activity did not derive more benefit from patient education than those experiencing low disease activity. On the contrary, an increase of anxiety and depression is found in these patients. Further study is needed to confirm our findings

Metode yang digunakan dalam penelitian ini menggunakan metode kualitatif. Teknik pengumpulan datanya dilakukan dengan cara memanfaatkan kepustakaan dan penelusuran data online. Hasil penelitian yang diperoleh adalah kewenangan yang dimiliki oleh Badan Pengawas Pemilihan Umum Kota Baubau, ketegasan dalam melakukan penindakan terhadap pihak-pihak yang melakukan pelanggaran, dalam menyelesaikan pelanggaran-pelanggaran yang terjadi selama pemilihan umum, Badan Pengawas Pemilihan Umum Kota Baubau telah melaksanakan tugas sesuai dengan amanat Undang-Undang yang berlaku.Kata Kunci : Pemilihan Umum Legislatif, Pengawasa

    Effects of Pair Creation on Charged Gravitational Collapse

    We investigate the effects of pair creation on the internal geometry of a black hole, which forms during the gravitational collapse of a charged massless scalar field. Classically, strong central Schwarzschild-like singularity forms, and a null, weak, mass-inflation singularity arises along the Cauchy horizon, in such a collapse. We consider here the discharge, due to pair creation, below the event horizon and its influence on the {\it dynamical formation} of the Cauchy horizon. Within the framework of a simple model we are able to trace numerically the collapse. We find that a part of the Cauchy horizon is replaced by the strong space-like central singularity. This fraction depends on the value of the critical electric field, EcrE_{\rm cr}, for the pair creation.Comment: LaTex, 27 pages, including 14 figures. Some points are clarified, typos corrected.     Personalized Medicine in Pediatrics: The Clinical Potential of Orodispersible Films

    Children frequently receive medicines that are designed for adults. The dose of commercially available products is adapted, mostly based on the child’s bodyweight, thereby neglecting differences in pharmacokinetic and pharmacodynamics parameters. If commercial products are unsuitable for administration to children or are unavailable, extemporaneous pharmacy preparations are a good alternative. For this particular population, orodispersible films (ODFs) can be a highly attractive dosage form for the oral administration of drugs. ODFs are relatively easy to prepare in a hospital setting, create dose flexibility, and may suit an individual approach, especially for patients having difficulties in swallowing tablets or being fluid restricted. In this article, various aspects related to pharmacy preparations, clinical application, and preparation of ODFs for pediatric patients are highlighted and discussed

    In this paper, some of the Turkish minstrel tales are analyzed in the frame of Gérard Genette’s ‘palimpsests’ approach. In the transtextuality category; the minstrel tales demonstrate both intertextual relations and hypertextual transformations. In terms of intertextuality, the tales present self-conscious intertextual relations by referring to other texts. The paper focuses on that in terms of hypertextuality, the mistrel tales transform the other texts by the process of reduction, extension, amplification and so on. Because of this process, the structure, the plot and the meaning of the previous text is transformed. As a result, the Turkish minstrel tales, as oral literary texts, can actively have role in intertextual relations as hypertexts.Türk halk hikâyelerine Fransız anlatıbilimci Gérard Genette’in “palempsest” imgesi ile yaklaşıldığında, halk hikâyelerinin ötemetinsellik sınıflandırmasında “metinlerarasılık” ve “ana metinsellik” ilişkilerini yansıttıkları görülmektedir. Halk hikâyeleri, göndermeler yoluyla bir başka metni somut olarak içinde barındırarak metinlerarasılık ilişkisi kurmaktadır. Bunun yanında, diğer sözlü ve yazılı metinleri biçimsel ve izleksel ya da anlamsal olarak dönüştürerek anlatısını yeniden kompoze etmek noktasında ana metinsellik ilişkisini kurmaktadır.     Quantifying similarity in animal vocal sequences: Which metric performs best?

    1. Many animals communicate using sequences of discrete acoustic elements which can be complex, vary in their degree of stereotypy, and are potentially open-ended. Variation in sequences can provide important ecological, behavioural, or evolutionary information about the structure and connectivity of populations, mechanisms for vocal cultural evolution, and the underlying drivers responsible for these processes. Various mathematical techniques have been used to form a realistic approximation of sequence similarity for such tasks. 2. Here, we use both simulated and empirical datasets from animal vocal sequences (rock hyrax, Procavia capensis; humpback whale, Megaptera novaeangliae; bottlenose dolphin, Tursiops truncatus; and Carolina chickadee, Poecile carolinensis) to test which of eight sequence analysis metrics are more likely to reconstruct the information encoded in the sequences, and to test the fidelity of estimation of model parameters, when the sequences are assumed to conform to particular statistical models. 3. Results from the simulated data indicated that multiple metrics were equally successful in reconstructing the information encoded in the sequences of simulated individuals (Markov chains, n-gram models, repeat distribution, and edit distance), and data generated by different stochastic processes (entropy rate and n-grams). However, the string edit (Levenshtein) distance performed consistently and significantly better than all other tested metrics (including entropy, Markov chains, n-grams, mutual information) for all empirical datasets, despite being less commonly used in the field of animal acoustic communication. 4. The Levenshtein distance metric provides a robust analytical approach that should be considered in the comparison of animal acoustic sequences in preference to other commonly employed techniques (such as Markov chains, hidden Markov models, or Shannon entropy). The recent discovery that non-Markovian vocal sequences may be more common in animal communication than previously thought, provides a rich area for future research that requires non-Markovian based analysis techniques to investigate animal grammars and potentially the origin of human language.We thank Melinda Rekdahl, Todd Freeberg and his graduate students, Amiyaal Ilany, Elizabeth Hobson, and Jessica Crance for providing comments of on a previous version of this manuscript.     Determination of the accuracy of implant reconstruction and dose delivery in brachytherapy in The Netherlands and Belgium

    Purpose: To gain insight into the accuracy of brachytherapy treatments, the accuracy of implant reconstruction and dose delivery was investigated in 33 radiotherapy institutions in The Netherlands and Belgium. Materials and methods: The accuracy of the implant reconstruction method was determined using a cubic phantom containing 25 spheres at well-known positions. Reconstruction measurements were obtained on 41 brachytherapy localizers, 33 of which were simulators. The reconstructed distances between the spheres were compared with the true distances. The accuracy of the dose delivery was determined for high dose rate (HDR), pulsed dose rate (PDR) and low dose rate (LDR) afterloading systems using a polymethyl methacrylate cylindrical phantom containing a NE 2571 ionization chamber in its centre. The institutions were asked to deliver a prescribed dose at the centre of the phantom. The measured dose was compared with the prescribed dose. Results: The average reconstruction accuracy was -0.07 mm (±0.4 mm, 1 SD) for 41 localizers. The average deviation of the measured dose from the prescribed dose was +0.9% (±1.3%, 1 SD) for 21 HDR afterloading systems, +1.0% (±2.3%, 1 SD) for 12 PDR afterloaders, and +1.8% (±2.5%, 1 SD) for 15 LDR afterloaders. Conclusions: This comparison showed a good accuracy of brachytherapy implant reconstruction and dose delivery in The Netherlands and Belgium

    Restricted Isometries for Partial Random Circulant Matrices

    In the theory of compressed sensing, restricted isometry analysis has become a standard tool for studying how efficiently a measurement matrix acquires information about sparse and compressible signals. Many recovery algorithms are known to succeed when the restricted isometry constants of the sampling matrix are small. Many potential applications of compressed sensing involve a data-acquisition process that proceeds by convolution with a random pulse followed by (nonrandom) subsampling. At present, the theoretical analysis of this measurement technique is lacking. This paper demonstrates that the ssth order restricted isometry constant is small when the number mm of samples satisfies m(slogn)3/2m \gtrsim (s \log n)^{3/2}, where nn is the length of the pulse. This bound improves on previous estimates, which exhibit quadratic scaling
